EMS System providers and members of the public may report incidents, complaints or concerns regarding prehospital care to the EMS Agency via the form provided below or via email (mary.vassar@sfgov.org) letter, fax (415-552-0194) or telephone call to the Quality Manager at 415-487-5042.
EMS System providers who are responding to a complaint may use the form provided below to report back to the EMSA on the findings of the complaint investigation.
