Incident Reporting

Exception, Commendation & Sentinel Event Reporting:

EMS System providers and members of the public may report incidents, complaints or concerns regarding prehospital care to the EMS Agency via the Exception & Sentinel form provided on the link below.  You may fax the form to (415) 552-0194 or call the Quality Manager at (415) 487-5042. Mail should be directed to:
